FPA GHV's Members Only "Nine & Dine" Event

It is hard to believe that our FPA GHV Chapter's Nine & Dine will be the first in-person event we've hosted in more than two years! And so it is our sincere hope that you will join us to play nine holes, followed by dinner and drinks at the Centennial Golf Club, a 27-hole championship course designed by Larry Nelson, a member of the World Golf Hall of Fame and winner of the U.S. Open and two PGA Championships. a 7,000-yard, par-71 championship golf course in Ossining, NY.

This is an opportunity to reconnect with your colleagues and get to know your Chapter President and other board members over the course of nine holes and dinner out on the patio (weather permitting).

Please arrive no later than 3:45pm so that we can get you teed up and ready to go!

By registering with the FPA of the Greater Hudson Valley (FPA GHV) to attend the golf outing on Thursday, April 21, 2022, at the Centennial Golf Club in Carmel, NY, I agree to abide by all health and safety guidelines and recommendations that may be in place at the time by federal, state and local government agencies and officials, FPA GHV, and the event venue. This may include, but is not limited to, submitting to restrictions set in place during the event that may consist of daily temperature checks, wearing a mask at all times in indoor public areas, engaging in appropriate physical distancing, and restrictions on certain activities that carry higher COVID-19-related risk, such as congregating in large private gatherings or attending off-site events in crowded facilities. I further agree that I will not attend this event in person if I become ill, am exposed to the COVID-19 virus within 30 days prior to the event, or exhibit any symptoms of the COVID-19 virus prior to the event. At present, we are not requiring proof of vaccination or negative COVID-19 test, although this requirement may change depending on federal, state and local government agencies and officials, FPA GHV, and the event venue safety recommendations that may be in place at the time of the event.

 

Additionally, I understand that travel to and participation in events may increase the risk of exposure to communicable diseases. I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE ANY AND ALL LIABILITY, CLAIMS, AND DEMANDS OF WHATEVER KIND OR NATURE AGAINST the FPA OF THE GREATER HUDSON VALLEY AND ITS AFFILIATED PARTNERS AND SPONSORS, INCLUDING IN EACH CASE, WITHOUT LIMITATION, THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, INDEPENDENT CONTRACTORS AND AGENTS (THE “RELEASED PARTIES”), EITHER IN LAW OR IN EQUITY, TO THE FULLEST EXTENT PERMISSIBLE BY LAW, INCLUDING BUT NOT LIMITED TO DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE, FAULT OR CONDUCT OF ANY KIND ON THE PART OF THE RELEASED PARTIES, INCLUDING BUT NOT LIMITED TO DEATH, BODILY INJURY, ILLNESS, ECONOMIC LOSS OR OUT OF POCKET EXPENSES, OR LOSS OR DAMAGE TO PROPERTY, WHICH I, MY HEIRS, ASSIGNEES, NEXT OF KIN AND/OR LEGALLY APPOINTED OR DESIGNATED REPRESENTATIVES, MAY HAVE OR WHICH MAY HEREINAFTER ACCRUE ON MY BEHALF, WHICH ARISE OR MAY HEREAFTER ARISE FROM MY ATTENDANCE AT THIS EVENT.
